The eyes have been described as the windows to the soul and are often the immediate focus when any two people first meet. We believe this is why we see so many patients at our Coral Gables and Miami clinic who are interested in refreshing and rejuvenating their eye (periorbital) area. These patients want more youthful eyes, and they want this usually without having to undergo a surgical procedure such as blepharoplasty (eyelid surgery).

When it comes to anti-aging, our preference is to focus on the full face. However, for patients who do not want or immediately need a complete facial treatment, we separate the face into 3 compartments and may choose to treat any one of these to get the best results. These compartments are: periorbital (around the eyes); perioral (around the mouth); and the midface (cheeks). Many of our patients choose to treat the eyes first as the fine lines and wrinkles, droopiness of the eyebrow, lowering of the eyelids, thinning of the skin, and undereye bags associated with periorbital aging can give them a tired look. These changes maybe seen as early as the third decade of life.

Once we decide that we are going to focus on nonsurgical eye rejuvenation, we then determine the best treatment approach. We may choose to treat dynamic wrinkles (those from muscle motion) with neuromodulators such as Botox, Dysport or Xeomin. We may also treat volume loss with fillers such as Juvederm, Restylane or Voluma. Lastly, each patient is recommended a topical skincare regimen that includes includes a sunscreen, antioxidant and retinoid. Some of our patients may also opt for the additional benefits of laser skin resurfacing with the Clear + Brilliant gentle laser or skin tightening with Ultherapy.

Common Signs of Periorbital Aging:

  • fine lines around the eyes
  • deeper, lateral canthal lines (crow’s feet) related to muscle activity of the orbicularis oculi
  • volume loss at the under-eye area seen as a prominent tear trough
  • shadowing under the eyes
  • hollowing of the region around the eyes
  • excess skin and lowering (ptosis) of the eyelids
  • lowering of the eyebrows
  • loss of elasticity
  • thinning of the skin
  • Nonsurgical Eye Rejuvenation Treatments:

Neuromodulators such as Botox are our go-to treatment for deeper lines and wrinkles around the eyes, particularly crow’s feet. Usually 16-24 units spread around both eyes will relax the muscle and soften the lines. Botox injected at the tail of the brow can also raise the eyebrow.

Fillers such as Belotero, Juvederm and Restylane are great for treating the undereye hollows and volume loss. Injections into the tear trough and along the lower orbital bone margin can improve undereye shadowing and hollowing. Actually, we can even see improvement in undereye shadowing when Juvederm Voluma is injected along the cheekbone. Finally, we may also use fillers such as Voluma to treat the temporal hollowing seen with aging.

While Botox can be used to raise the eyebrows, a skin tightening treatment such as Ultherapy can also open up the eyes, tighten the skin and to raise the brow. With Ultherapy, the results are longer lasting (up to 3 years) and with the newer treatment protocols, the procedure is almost pain free.

We always start our patients on a skincare regimen which usually includes an antioxidant, sunscreen and retinoid. Prescription strength retinoids such as Retin-A or over the counter retinoids such retinol or retinaldehyde from Skinceuticals or Avene can improve the tone and texture of the skin, eliminate fine lines, increase collagen, reduce pigmentation and tighten skin. If the patient has more advanced signs of aging or sun damage, then we may also offer to treat the skin with a fractionated laser treatment using Clear + Brilliant gentle laser in additional to the skin care. Chemical peels such as a glycolic acid peel can exfoliate the skin and at higher strengths actually remodel the skin and improve overall skin health.

Our goal with the above treatments is to eliminate fine lines and wrinkles, replace lost volume, and soften undereye shadowing, while also tightening and lifting the skin. Once we achieve this periorbital rejuvenation, we help patients maintain their beautiful results through the use of a broad spectrum SPF 50 sunscreen as well as a topical antioxidant (our favorite is SkinCeuticals C E Ferulic) and retinoid.
